At Motivate Home Services we are committed to creating individually designed plans of care for our clients. Our client focus is on clients that are on the CADI and BI waiver programs. The services we provide include, but are not limited to the following:

• Mental Health and Medical Nursing Visits

• Case management, Care Planning

• Coordination with mental health and medical providers

• Coordination with support team: case workers, ILS workers, ARMHS workers, guardians, family

• Referrals to community resources

• 24-hour awake supervision and oversight

• Assistance with personal care like bathing, dressing and grooming

• Assistance with mobility: walking, transferring, exercise plans

• Three meals per day, snacks, feeding if necessary

• Nursing sets up all medications for aide staff to administer

• Obtaining medication orders and medications from the pharmacy

• Appointment management

• Transportation to appointments or telehealth

• Nebulizers, CPAPs, Oxygen

• Accuchecks, Insulin Administration, and sliding scale insulin

• Catheters and Ostomies

• Feeding Tubes

• Wound Care

• Assistance with money management

• Behavioral Management
